Membership, Cancellation & Refund Policy

This page summarizes how Sparkle Pediatrics Direct Primary Care membership works in relation to insurance, cancellation, and fees. Your signed Patient Membership Agreement controls if there is any conflict.
Important Notice Regarding Direct Primary Care
Sparkle Pediatrics is a Direct Primary Care (DPC) practice. This membership program is NOT health insurance and does not meet the minimum essential coverage requirements under state or federal law.
DPC membership covers specified primary care and pediatric services provided directly by Sparkle Pediatrics. It does not cover specialist visits, emergency room care, hospitalizations, advanced imaging, or outside laboratory services. We strongly encourage all patient families to maintain comprehensive health insurance or a catastrophic health policy to cover medical needs outside the scope of our primary care services.
How to cancel
To cancel a membership, email info@sparklepediatrics.com or call 904.333.2896. Please include the member child’s name and the parent/guardian contact information associated with the account.
Notice period
We require 30 days’ notice to cancel a membership. Cancellation becomes effective at the end of the notice period unless your membership agreement states otherwise.
Refunds
Prepaid monthly membership fees are non-refundable. This includes fees already paid for the current billing period and any period covered during the 30-day notice window.
Re-enrollment
There is no re-enrollment fee to rejoin Sparkle Pediatrics after a prior cancellation, subject to practice capacity and acceptance of a new membership agreement at the then-current rates and terms.
